When we receive a referral we will check to ensure that our service is the most appropriate for your need.

You will usually be referred by your consultant or GP to one part of EllenorLions Hospices. You may subsequently be referred to other parts, if your needs change.

If your needs reduce or you no longer require support from our service, you may be discharged to the care of your GP. If your needs change again later, you can be referred back to us at any time.

Care is provided in various settings, including your home or nursing home, the local hospital and at our EllenorLions sites in Dartford and Northfleet. Staff will try to uphold your personal preference for place of care, but this is not always practicable. In all cases, staff work closely with you, your family and colleagues from health and social services to help find the right place.
