The EllenorLions Palliative Care Support Team have been commissioned and funded by the West Kent PCT to provide an End Of Life Care Crisis Support Service for Dartford, Gravesham and Swanley.
In a crisis situation, the service provides a 24/7 rapid response service in the home, for people at the end of their life; and for their families.
Where people?s preferred place of choice to die is in their own home, this will enable them to do so by providing practical hands-on care and emotional support. The service will concentrate on cases where the family/carers require additional support to avoid an admission to hospital; or alternatively to facilitate a rapid discharge home, including from A&E.
The service is available to people who are not existing EllenorLions patients if a Health Care Professional has assessed them to be at the end of their life.
